Youth Work Permits
If you are under 18 years of age, you must obtain a work permit before starting a new job. Additionally, a Certificate of Health is required only for 14 and 15-year-olds.
Steps for getting an employment permit:
Print and complete the employment permit application. If you need a printed copy, please come to the Enrollment Services Center at 57 Suffolk St.
Ask the employer who has made the job offer to complete the Promise of Employment section of the permit application.
For 14- and 15-year-olds only (16- and 17-year-olds may skip this step): Ask your doctor to complete the Certificate of Health section of the permit application.

Once the permit application is complete, it can either be brought to the Enrollment Services Center in person, or can be emailed to enrollment@hps.holyoke.ma.us.
An Enrollment Center staff member will provide the youth with a work permit to bring to their new employer.
Please note, all youth that do not attend Holyoke Public Schools will need to provide a birth certificate, passport, or other legal document with their date of birth.
